At-home palliative care and terminal care are aimed at helping people with those with little life expectancy due to illness, including people with dementia and old age, to spend the rest of their lives on their own, being satisfied and ending their lives.

However, it is difficult to realize it because of the heavy burden on the caregiver family, anxiety about how to respond to sudden changes in the condition, and the absence of a doctor attending the home visit.

Also, the psychological state of a person who is told that he does not have much time left due to illness is extremely unstable. The anger of “why me?”, unavoidable resistance to death, fear, and anxiety are often expressed as words and actions, and this is a difficult time for family members.

Most Japanese people wish to die in their own home surrounded by their family, but the desire has not been fulfilled for various reasons. The cooperation of supporting families, healthcare workers, and local communities will be essential.

Finding the problem

Then, what is necessary for health care workers and the community to support the person and family when providing palliative care at home?

Argument

When performing palliative care at home, the burden on the family will be significantly huge, and thus a reliable system is necessary.

First of all, in community medicine, it is important that doctors and nurses have sufficient knowledge and skills for palliative care, and that family members can receive appropriate treatment at home visits.

In fact, as a result of conducting surveys on palliative care at home for medical institutions, it has been found important to improve knowledge of palliative care for doctors working at community clinics and to build a cooperative system with specialists on the hospital side that supports them.

Also, in the community, it will be necessary to enhance care services and home-visit nursing systems in order to reduce the burden on families.

Conclusion

Therefore, I would like to investigate and understand the current status of medical institutions and their local services in home palliative care, and build a system that allows families who actually support patients to acquire sufficient knowledge about home nursing and select home palliative care.
